- Statutory declarations, like statutory declarations of marital status, statutory declarations of common law status, applications and statutory declarations of identity
- IMM 0006 Application for Authorization and Statutory Declaration
- Sworn statements for a family gift of a used vehicle
- Letters of invitation
- Ontario court forms, including affidavits of service and affidavits of divorce
- Separation agreements
- Law Society of Ontario Administrative Call application
- Statutory Declaration such as Custodianship, Common Law, OSAP, Progress Payment
- Proof of Loss Form
- Consent to Travel letter
- Name Change Application
- Delayed birth registration forms;
- Application to amend birth registration;
- Declaration affirming parentage;
- Election to change of name of child under the age of 12;
- Statutory declaration by an applicant to correct an error in registration;
- Legal name change application;
- Blank statutory declaration;
- Insurance claims;
- Common law status affidavits;
- Special invitations to family members overseas;
- Passport applications (in lieu of guarantor);
- Pension purpose attestations of recipient being alive and confirming their residency;
- Residency document;
- Lost passport;
- Single status certificate;
- Consent letter for traveling with a child commonly referred to as a travel document;
- Federal government permanent residency card applications
- Municipal Information Form (MIF) for liquor license;
- Affidavit waiving vaccinations based on religious grounds.
- Any other Statutory Declaration or Affidavit that needs to be commissioned
The following is a list of documents acceptable to verify your identity:
- Driver’s License
- Passport- Canadian
- Passport- Foreign
- Canadian Citizenship card with Photo
- Canadian Permanent Resident Card with Signature
- Confirmation of Permanent Resident (IMM 5292) accompanied by a valid passport from a country of origin
- Record of Landing (IMM 1000) (some Exceptions apply)
- Student Authorization- Study Permit (IMM 1442)
- Employment Authorization/ Work Permit (IMM 1442)
- Secure Certificate of Indian Status Card – issued on or after December 15, 2009, by Indian & Northern Affairs Canada
- Temporary Resident’s Permit (IMM 1442)- formerly Minister’s Permit/Extension of Minister’s Permit
- Visitor Record
- Driver’s licence or enhanced driver’s licence – issued by another Canadian or U.S. jurisdiction Ontario driver’s licence or enhanced driver’s licence
- Canadian Department of National Defence (DND) identity card
